
Do You Need a Referral for Physical Therapy in Ohio?
Short answer: no. Ohio is a direct access state, which means you can see a physical therapist without a referral or prescription from a doctor. You can call us, book an evaluation and start PT the same week.
A lot of people assume they have to see a doctor first, wait for that appointment, and then get told to go to physical therapy. In Ohio, you can skip the middle step. Here's how it actually works.
What does the law say?
Ohio's direct access law (Ohio Revised Code 4755.481) lets physical therapists evaluate and treat patients without a referral. It comes with a few rules, and honestly, they're good ones:
- ✓With your OK, we let your doctor know we evaluated you, within five business days.
- ✓If you haven't made substantial progress within 30 days of your first visit, we have to consult with or refer you to a physician or another licensed provider. (There are exceptions, like fitness and wellness training.)
- ✓If anything points to a problem outside what physical therapy can treat, we refer you out.
That last one matters. Part of every evaluation is screening for things that aren't a PT problem. Spotting those red flags is part of what a Doctor of Physical Therapy is trained to do, and if I see one, you'll hear it from me the same day.
When should you see a doctor first?
Direct access doesn't mean PT is always the right first stop. See a doctor first, or go to the ER, if you have:
- ✓A fall, crash or big hit, and you can't put weight on it or something looks deformed.
- ✓New numbness in the groin or inner thighs, or trouble controlling your bladder or bowels. That's an ER trip.
- ✓Chest pain or shortness of breath, or a swollen, painful calf, especially after surgery or a long trip. Get seen right away for those.
- ✓Back pain with a fever, or with weight loss you can't explain.
- ✓Weakness that's spreading or getting worse fast.
- ✓Pain that's constant, keeps getting worse, and doesn't change at all no matter how you move or rest.
If you're not sure, call us. We'll tell you honestly whether to come in or see your doctor first.
What about insurance?
Here's the catch. The law lets you see a PT without a referral, but it doesn't make insurance companies pay for it. Some plans still want a referral before they'll reimburse anything.
At Big Cat we're cash-pay, so you never need a referral to see us. We take HSA and FSA cards, and if you want to try for out-of-network reimbursement, we'll give you a superbill. Just ask your plan first whether it needs a referral before it'll pay you back.
What if I'm coming off surgery?
You don't need a separate referral to see us after surgery either. Bring your surgeon's protocol so we're all on the same page, and we'll keep your surgeon in the loop as you progress.
